Sadly, Neil Young & Crazy Horse were forced to cancel their remaining tour dates due to a hand injury to Frank "Poncho" Sampedro. The shows cancelled are:

8/31 Dundas, ON - Greenbelt Harvest Picnic
9/02 Port Chester, NY - Capitol Theatre
9/04 Ottawa, ON - Ottawa Folk Festival
9/07 Arrington, VA - Interlocken Music Festival

Drummer Ralph Molina made the following statement on his facebook:

Following that, Bass player Billy Talbot made a statement through Neil Young fan site, Thrasher's Wheat:

All four of us in the Horse are extremely disappointed in this turn of events. We've never had to cancel a show before. After being together for 40 years, we know there's no way we can play without Poncho, or any one of us. The four of us have to be there for it to be NYCH. If there's any way that we could have played these shows, we would be there. This is an extremely hard twist of fate for us to accept.

One of the reasons that we love what we're doing and it rocks so much, is the fact that we respect that it comes from the four of us. A unified passion.

It's good that the band is being positive about the unfortunate series of events that's led to this.

Earlier this year, before the injury Poncho told Rolling Stone magazine:

My gut tells me this is really the last tour. I hate saying their ages, but I'm 64 and I'm the baby of the band. I love playing and we're playing as good as we ever did, but at any time something could go down with any one of us.

Since the injury cut the tour short, it's reasonable to think that Neil Young & Crazy Horse will be playing again. Only time will tell when that happens.
